The Endothelin receptor antagonists

To have a view of the treatment of pulmonary hypertension with Endothelin receptor antagonists we must have knowledge of Endothelin-1 (ET-1). It is a 21 amino acid peptide produced by the endothelium of blood vessels. Being a very potent vasoconstrictor this peptide binds to smooth muscle endothelin receptors. There are two subtypes ETA and ETB receptors. The receptor activation will lead an increased smooth muscle contraction and vasoconstriction. The mainstay of the treatment is to block the activity of this Endothelin-1. All the drugs in this category will however block the receptors of this peptide. These can be selective for either ETA or ETB or affects both.

Bosentan

was approved in the 2001, more or less for the specific treatment of pulmonary hypertension. This Endothelin receptor antagonist drug is marketed under the name Tracleer. It acts as an endothelin receptor antagonist whereby blocking the interaction of endothelin-1 binding to ET-A or ET-B receptors which otherwise causes pulmonary vasoconstriction. By blocking this interaction, this drug decreases pulmonary vascular resistance. Bosentan has a slightly higher affinity for ET-A than ET-B nonetheless.

Sitaxentan

the next drug is of sulfonamide class endothelin receptor antagonist which is a selective endothelin receptor antagonist that blocks only the interaction of ETA to endothelin-1. It is approved for use in Canada, Australia, and the European Union under the name Thelin. But this is not the case in the US where it is not yet approved for marketing by the U.S. Food and Drug Administration (FDA) is undergoing review for treating pulmonary hypertension.

Ambrisentan

is another similar drug which is marketed as Letairis. It is indicated for the treatment of pulmonary arterial hypertension categorized as WHO Group 1 that is in patients with class II or III symptoms and used to improve exercise capacity while having an effect of delay in clinical worsening of these symptoms.
While more is being done elsewhere, another dual/nonselective endothelin antagonist, which is known as Actelion-1, it is a product of the makers of Tracleer known as Actelion Pharmaceuticals and was entered in the clinical trials in 2008.

Epoprostenol
Epoprostenol is the synthetic prostacyclin which is marketed as generic Flolan. This medication is infused continously but not without cons. Being a Prostacyclin I2 analogue it can be administered PO. However it is designated as an orphan drug in the United States. It causes the Pulmonary vascular dilation secondary to an increased cyclic adenosine monophosphate (cAMP) while Inhibits the platelet aggregation.
